Free Quality of Life Assessment for Pet

Thinking about Pet Euthanasia but your not sure its the right time?

We know the most difficult decision you ever have to make is if it "the right time". As pets grow older their needs naturally change. Mobility becomes more difficult or painful, and long-term illnesses or age-related conditions can begin to affect their daily comfort. In some cases, administering medication or managing ongoing treatment may also become challenging or costly. Even routine trips to the vet can cause significant stress or discomfort for your pet.

We have created a free quality of life assessment for your pet, please use it as many times as you need - But PLEASE remember it should only be used as a guide to help you and you should never make the decision based solely on this. This is totally free - you do not have to register or leave any personal details.

Our Pets Quality of Life Assessment will ask you 11 questions over multiple areas of your pets wellbeing, including Mobility, Nutrition, Hydration, Interaction & Attitude, Toilet Habits, Favourite Activities.

Every pet is unique. Think about the things your pet loves most if its playing with toys, going for walks, relaxing in the garden, or simply being close to you. Are they still showing interest in these activities? This is a deeply personal but essential part of assessing their quality of life.

The scale we use is 0 - 10 where 0 is Very Poor, 5 being Average and 10 being Excellent

We hope this helps and hope that you do not need our services anytime soon but if you do you can contact us on 01200 401070, drop us a whatsapp message or email us if you would like us to give you call.

This survey is adapted from validated veterinary Quality of Life (QoL) instruments and hospice scales.

Please confirm you have read the disclaimer below to proceed with the survey.

If the total score is low, or if any single domain is very low, you may want to discuss with your own vet for help in making your decision.

You will need to acknowledge our disclaimer before using the Quality of Life Assessment

Important Notes

  • Emergency: If severe respiratory distress, uncontrolled pain, inability to eat/drink, contact your vet urgently.
  • Low single scores: A single very low score for any question (e.g. severely painful, not eating, struggling to breathe) may outweigh the total score and you should discuss with your vet.
  • Repeat: Repeat weekly or sooner. If condition changes, share results with your vet.

Action Plan

Please feel free to share this completed survey with your vet. If unsure, you should discuss this with your vet. You should inform your vet that you have used our QoL assessment, which is here as a helpful guide only and not to determine medical conditions or a sole basis for your decision.
